Preferred Provider Organization

A Preferred Provider Organization, PPO, is a network of health care providers from whom you can choose to receive health care services. A PPO contracts with physicians and hospitals to secure preferred rates for you and your covered family members. You can decide to visit a PPO provider any time medical care is needed. When you select a ConnectCare PPO participating provider for your medical care, your share of the out-of-pocket cost of those services is reduced, based on the benefits of your medical plan. Eligible expenses are covered no matter what provider you select for your medical care; however, your share of the cost is greater if you choose a non-network physician.
