For care needs beyond what is locally available, ConnectCare, through an agreement with Preferred Provider Organization Midwest (PPOM), makes available over 55,000 additional physicians and 460 hospitals throughout Michigan, Ohio, Indiana, and in parts of Kentucky and Wisconsin.
