ConnectCare offers the largest local network of physicians and hospitals, in the mid-Michigan area, as well as an extended network of participating hospitals and other services. We can help you connect with doctors, hospitals, and a large number of other health care providers located throughout the state. We also provide online search capabilities and maps to get you to your destination.

It is your responsibility to verify whether your health care provider currently participates in the ConnectCare network before you obtain health care services.
Ask whether the provider participates with ConnectCare, rather than if he/she accepts ConnectCare. If your employer has made the Cofinity network available to you outside the local area, you will need to verify Cofinity participation in the same way. Providers may indicate a willingness to accept ConnectCare or Cofinity even though they don't participate, which means your out-of-pocket expenses will be greater than if you choose a participating provider.
For services from other health care providers such as laboratories, x-rays, or for medical supplies, please ask your physician to refer you to a participating ConnectCare or Cofinity provider. However, remember that it is your responsibility to verify participation for all services.
